Mandatory High School Athletic Forms for All Sports

Select HERE for the Pinewood School Athletic Code Agreement, which includes the Athletic Code of Conduct, Parent Code of Conduct, Athletic Expectations, Risk Warning, Supervision Release, and Acknowledgement.


Select HERE for the Athletic Pre-Part Screening Exam form, which covers an athlete's General Information, Health History, Parent/Guardian Acknowledgement on page 1 along with an athlete's General Examination Results and Physician Recommendation on pages 2.


Select HERE for the Pinewood High School Athletic Participation Card which covers an athlete's Parent/Guardian Consent, Physician's Statement, Emergency Contact information, and Insurance Requirements.


Select HERE for the Age Requirement Waiver. This form is required for all athletes who wish to compete at the varsity level playing Tackle (8 Man) Football and are under the age of 15 years old. The form includes the Licensed Medical Practioner Release Statement, Parent Consent, and Head Coach Consent
